Make a Payment and Order Materials

Include with your payment: Your Client Number (found on the back of your chart) or your First Name, Last Initial AND your Practitioner's first name. We have multiple practitioners at GFCC and we want to ensure that your payment is applied properly.

Please read BEFORE your order materials: This is for active Groesbeck FertilityCare Clients to order more charts and stamps. We can only sell materials to clients who are seeing a GFCC practitioner and have had a follow up within the last 6-12 months or have one coming up in the next month.
